Dementia Consultation
(2-3 hours)
Our Dementia Consultation provides a brief evaluation to rule out dementia-related conditions, The evaluation includes a thorough clinical interview, interview with family member(s), brief cognitive testing, and a discussion of the results. The purpose of this evaluation is primarily to answer the question of: Does this person have a dementia?
Memory Evaluation
(4-6 hours)
Our Memory Evaluation service is designed to provide a thorough assessment of memory and thinking abilities to determine whether there are indeed signs of cognitive impairment (e.g., dementia, etc.). Dementia is a complex diagnosis that involves impairment of memory, other thinking abilities, and day-to-day activities. Sometimes a brief evaluation is not enough to differentiate from other causes of thinking problems. Timely evaluation and diagnosis are crucial to ensure appropriate care and support.
Independent Living Evaluation
Schedule an Independent Living Evaluation with DAS and get an informed opinion on whether your loved one should be on their own. Our evaluation takes into account an individual’s cognitive functioning and capacity for independent living and decision making. We will work closely with you and your loved one to ensure that your individual needs and preferences are met. Let us help you make the right decision for your family.
